Your image gets printed directly onto a sheet of 1/16" thick aluminum. The aluminum sheet is offset from the wall by a 3/4" thick wooden frame which is attached to the back. This door in Exeter Cathedral is below the Astronomical Clock and gave access to the workings including many ropes. Mice chewed the ropes so a cat was hired to catch the mice hence the Cat Flap thought to be the first.
